Membership Terms & Conditions:
- The first Botox®/Dysport® treatment included with your membership becomes available after your third monthly payment has been received.
- Thereafter, your included Botox®/Dysport® treatment may be redeemed every three (3) months while your membership remains active and in good standing.
- Monthly membership payments will be automatically charged to the card on file.
- Membership benefits are non-transferable and may not be shared, assigned, or gifted to another person.
- No long-term contract is required.
- To cancel your membership, we require a minimum of thirty (30) days written notice submitted via email.
- Membership fees are non-refundable.
- Unused membership benefits do not roll over and have no cash value.
